D2 follows Laura, a plane-crash survivor who awakens in a mountain cabin with no memory of her identity. As mutated crash survivors and locals begin attacking, she must investigate the frozen Canadian wilderness and uncover the mystery behind the catastrophe.

The game combines horror, adventure, shooter, and role-playing elements. Players can freely navigate the snowy landscape, explore interior locations from a first-person perspective, pilot a snowmobile, hunt for food, and take photographs. Story-focused cutscenes are triggered while exploring and investigating the environment.

Monsters appear randomly or as plot-specific bosses, moving combat to separate shooting sequences. Laura must react quickly to enemies attacking from different directions, using an unlimited-ammunition default gun or stronger weapons with limited ammunition. Defeating enemies grants experience points, allowing Laura to level up, improve her strength, and increase her hit points.
